Required Health Information Health Insurance

All students residing in any/all university provided housing and all international students attending classes on an F1 or J1 visa, are required to comply with the University's Health Policy and present the following information to the Student Health Center.

Application Forms

Application Fee Waiver Form

The nomination form for alumni to recommend potential students for admission to Barry University. This form will waive the application fee.

Authorization for Release of Application Information

For use by applicants who wish to permit an agent or representative to receive information on their application status.

Change of School/Program Form

For enrolled students who wish to change from one school or program at Barry University to a different school or program at Barry.

Graduate Readmission Application

To be used by students who have been inactive in their academic program for one calendar year.

Non-Degree Application

For applicants wishing to enroll as guest students, non-degree students, or those pursuing teacher (re)certification.

Photography Portfolio Review for MA and MFA Degree Program

Must be submitted with Graduate Application for the MA/MFA in Photography.
